Tapestry Day Club is the trading name of Tapestry Lunch Club CIC. By completing and signing this form, you give your consent (if appointed) for Tapestry Lunch Club CIC, under the 2018 General Data Protection Regulation, to retain and to make reasonable use of your personal data for the purposes of your contract with Tapestry Lunch Club and for Tapestry Lunch Club to communicate with you by email, mail and phone.
